If you wish to make a Privacy Rights Request, please complete the form below to ensure your Request is processed in the most efficient manner. Please double check and confirm that the information entered below is accurate to prevent a delay in responding to your Request.

Upon receipt of your Request, we will provide directions on identity verification requirements which may include your name, email address, and/or information Kandy maintains on you, where appropriate. Your request will not be processed until your identity has been verified. Once your identity has been confirmed, your Request will be processed in accordance with applicable laws and regulations.

You may choose to designate an authorized agent to make a request on your behalf. No information will be disclosed until the authorized agent’s authority has been reviewed and verified. Once a request has been submitted by an authorized agent, we may require additional information (i.e., written authorization from you) to confirm the authorized agent's authority.

If you are an Authorized Agent, please be advised that the data subject may be contacted to confirm that they have authorized you to make the submitted request.
